What is a membership? A membership with Vial Family Dentistry is an annual wellness plan for families and individuals.VFD Dental Membership allows all members to receive quality dental services at significantly reduced prices. There are NO deductibles, NO pre-authorizations, NO maximums, NO missing tooth clause, NO material downgrades, and NO waiting periods.

Preventative Membership

Benefits Include in a 12 Month Period:

✔ 15% off on all Basic and Major Dental services
✔ Discounts do not apply to medications or supplies


VFD Dental Membership is only $399 for a single patient. $750 for 2 members in the same household, $300 for any additional family member in the same household under the age of 26.

Periodontal Membership

Benefits Include in a 12 Month Period:

✔ 2 Check-ups
✔ 3 Perio maintenance cleanings, Get a 4th for $75
✔ Routine Checkup X-rays
✔ 2 Fluoride Varnish Treatments
✔ 15% off on all Basic and Major Dental services
✔ Discounts do not apply to medications or supplies


VFD Dental Membership is only $500 for a single patient. $950 for 2 members in the same household.



Program Guidelines

  • Payment is due in full at the time of service to receive the benefit. A missed appointment fee of $55 may be charged for any missed dental appointment without a 24-hour cancellation.
  • VFD Dental Membership is a dental wellness plan, not dental insurance, and cannot be issued in conjunction with dental insurance, state funded, or any financing program such as CareCredit.
  • Benefits not used are not carried over to the next enrollment period. Plan is non-transferable.
